All Allen Academy families need to have the following forms on file. All of these forms are available online and are a part of the enrollment process. Thus, if you've successfully enrolled at Allen Academy, all of these forms are on file.

Families with students that will be participating in athletics need to complete physical forms and the TAPPS Acknowledgement of Rules form. These forms can be found on the Athletics page.
